Medi-Cal, Medi-Cruz, Central Coast Alliance for Health and self-pay clients are accepted. Same day appointments on a limited basis for established clinic adult and pediatric patients with urgent medical problems. Clinics offer laboratory, pharmacy and x-ray services for clients at both sites. Services include: immunization and Tuberculosis clinics. Medi-Cal, Medicare, and Medi-Cruz accepted. Lab, X-Ray, and pharmacy services also available. Santa Cruz location: 1080 Emeline Ave. Santa Cruz See link below in related agencies. ELIGIBILITY: Open to all, but priority is given patients eligible for Medi-Cruz and Medi-Cal. FEES: Fixed fee. Medi-Cruz for those eligible, Medi-Cal and Medicare accepted. AREAS SERVED: Watsonville and all south Santa Cruz County and northern Monterey County. |
Programs: | "Breast Cancer Early Detection Program (BCEDP): Clinical breast examination and referral for mammogram for women 40 years of age and older. Free service for eligible clients. Child Health and Disability Prevention (CHDP): Well Child examinations, immunizations, referral to specialty care for infants, children and teens. No cost to eligible clients. Family Planning Services: Annual PAP exams, family planning counseling and information. Emergency contraception available, no appointment needed, same day service. Homeless Persons Health Project: provides services throughout Santa Cruz County to homeless or marginally housed youth, adults, and families with children. See link-related agencies below Immunization Services: Immunizations and TB testing for infants, children, teens and adults. Integrated Community Health Center Commission: The purpose of the County of Santa Cruz Integrated Community Health Center Commission is to act, in conjunction with the Board of Supervisors, as the governing body of those health care clinic operations of the County which are designated as Federally Qualified Health Centers (""FQHCs"") pursuant to 42 U.S.C. Section 1395x(aa)(3) and/or 1395x(aa)(4) (FQHC clinics) and 42 U.S.C. Section 254b(a) (Health Centers). The commission consists of 11 members appointed by the County of Santa Cruz Board of Supervisors. Orthopedic services: Orthopedic services (diseases and problems with your bones) for Medi-Cruz and Medi-Cal clients by referral of the Central Coast Alliance for Health provider only. Offered at Santa Cruz Clinic only. Pediatric Services: Pediatrician directed clinic for ill infants and children. Watsonville Clinic ONLY. Primary Care and Urgent Care Services: Medical care for individuals and families. Medi-Cal, Medi-Cruz, Central Coast Alliance for Health and self-pay clients are accepted.
